Último número de The Hematologist comenta sobre reunión de la ASH en Cuba

La Revista The Hematologist es un órgano de noticias y reportes de la Sociedad Americana de Hematología  (ASH, por sus siglas en Inglés), y en su último número, el Presidente de la ASH, Prof. Dr. Charles Abrams, en su columna “Open Borders, Open Minds” defiende la idea de la realización de la reunion del ejecutivo en La Habana, y comenta favorablemente sobre el Intercambio Educacional con los miembros de la Sociedad Cubana de Hematología. Por su parte, el Dr. Jason Gotlib, profesor asociado de hematología de la Escuela de Medicina de la Universiad de Stanford/Instituto del Cáncer, y Director del Programa de Becas de Hematología de dicha Universidad,  presenta su artículo títulado “ASH in Cuba: Unmasking Challenges and Opportunities in Hematology”, donde realiza un análisis de la situación en Cuba y el desarrollo de la Hematología,  con los aportes hechos por el IHI y la SCH.

Targeted gene editing restores regulated CD40L function in X-linked hyper-IgM syndrome
In their plenary paper in this week’s Blood, Hubbard et al provide an innovative approach of genome editing to knock in a wild-type cDNA and functionally correct disease-causing mutations in a gene in primary human T cells. The level of gene editing appears high enough to potentially allow therapeutic correction of primary immunodeficiency.

VTD is superior to VCD prior to intensive therapy in multiple myeloma: results of the prospective IFM2013-04 trial
In an important clinical trial, Moreau et al compared bortezomib-thalidomide-dexamethasone (VTD) to bortezomib-cyclophosphamide-dexamethasone (VCD) as induction before high-dose therapy and autologous stem cell transplantation in patients with newly diagnosed multiple myeloma. The ramifications are broad. Their data show that the VTD regimen is markedly superior for newly diagnosed myeloma.

Exome sequencing reveals recurrent germ line variants in patients with familial Waldenström macroglobulinemia
In this issue of Blood, Roccaro et al report the possible contribution of the germ line genetic variants LAPTM5c403t and HCLS1g496a to inherited susceptibility to familial Waldenström macroglobulinemia.

Welcoming a new age for gene therapy in hematology
The exciting new field of genome editing represents a quantum leap for biology and medicine. A series of four reviews written by experts in the field summarize conceptual, clinical, and ethical aspects of genome editing with a focus on its potential for treating hematologic diseases.

The articles in this review series, “Genome Editing in Hematology,” include the following:

Role of platelets, neutrophils, and factor XII in spontaneous venous thrombosis in mice
Heestermans et al examined the roles of platelets, neutrophils and FXII in a new genetic mouse model of spontaneous venous thrombosis. Their data profoundly challenge the current notions about these players in venous thrombosis.

Novel phosphatidylethanolamine derivatives accumulate in circulation in hyperlipidemic ApoE−/− mice and activate platelets via TLR2
Biswas and colleagues show that a group of oxidized lipid species are a new type of platelet agonists that induce platelet activation via a TLR2 receptor pathway. They induce cross talk between innate immunity and integrin activation signaling pathways. These data are directly relevant to the atherothrombosis field.

The 2016 revision of the World Health Organization classification of lymphoid neoplasms

The 2016 revision to the World Health Organization classification of myeloid neoplasms and acute leukemia
The most recent World Health Organization classification of Tumours of Haematopoietic and Lymphoid Tissues was published in 2008. The intervening 8 years have seen an explosion of new genetic information about hematopoietic neoplasms that remain to be integrated into diagnostic algorithms for these diseases. Swerdlow and colleagues and Arber and colleagues summarize the revision to the prior classification that incorporates new clinical, prognostic, morphologic, immunophenotypic, and genetic data.

Phase 2 study of idelalisib and entospletinib: pneumonitis limits combination therapy in relapsed refractory CLL and NHL
Barr and colleagues report the outcome of a phase 2 study of the combination of the phosphatidylinositol 3-kinase δ inhibitor idelalisib and the spleen tyrosine kinase inhibitor entospletinib for the treatment of refractory chronic lymphocytic leukemia and non-Hodgkin lymphoma. Although patients had significant responses, the study was terminated early because of treatment-related pneumonitis occurring in 18% of patients with 2 deaths.

Early cytomegalovirus reactivation remains associated with increased transplant-related mortality in the current era: a CIBMTR analysis
Teira and colleagues report a retrospective analysis of the impact of cytomegalovirus (CMV) reactivation on relapse and survival in over 9000 recipients of hematopoietic stem cell transplants. Despite current better treatment regimens directed at the virus, CMV reactivation remains a risk factor for transplant-related mortality. Furthermore, in contrast to some previous studies, CMV is not associated with decreased leukemia relapse.

Rapid discrimination of the phenotypic variants of von Willebrand disease

Clinical and laboratory variability in a cohort of patients diagnosed with type 1 VWD in the United States
The diagnosis of VWD subtypes remains somewhat challenging, as highlighted in two articles in this week’s issue. In this month’s CME article, Roberts and colleagues report on the development of an enzyme-linked immunosorbent assay–based assay that accurately diagnoses phenotypic subsets of von Willebrand disease (VWD) with over 85-95% accuracy. This rapid test may streamline the appropriate diagnosis of patients with VWD variants. In a second article, Flood and colleagues examine the correlations among the bleeding assessment tool, von Willebrand factor (VWF) levels, and detectable mutations in VWF genes in 482 individuals carrying a diagnosis of type 1 VWD. They report that VWF levels do not predict for bleeding scores, but low VWF levels predict for sequence variants in the VWF gene.

Comprehensive mutational profiling of core binding factor acute myeloid leukemia

RNA-sequencing analysis of core binding factor AML identifies recurrent ZBTB7A mutations and defines RUNX1-CBFA2T3 fusion signature
Two publications expand our understanding of the genetics of core binding factor acute myeloid leukemia (AML). In the first, Duployez and colleagues performed deep sequencing of 40 known myeloid malignancy–associated genes in over 200 patients with t(8;21) and inv(16) AML, demonstrating that the patterns of mutations in the 2 subtypes are different and pinpointing mutations predictive of poor prognosis in t(8;21). In a letter to Blood, Lavallée and colleagues describe an untargeted RNA-sequencing analysis of 48 samples, identifying novel mutations that could not be predicted by the targeted sequencing approach.
